If applying for Food assistance make sure to send a copy of your photo ID as it must match the name on the application. This is how we cut down on fraud.

If applying for Financial Assistance make sure to send a copy of your ID along with a denial from Care Credit and Scratch Pay. We do ask that you put something toward the care of your pet as well.
